Pain Anesthesia Fellowship

Hospital Hamilton Health Sciences Corporation, McMaster University
Description Approximately 500 chronic pain patients per month, 10 to 20 cancer visits per month, 120 acute pain inpatients per month.

Acute pediatric experience available.

Services directly available in the program include: Psychology, Nursing, Physical Medicine, Rehabilitation Medicine, Psychiatry,
Exercise Physiology, and Vocational Rehabilitation (Ethics). Ongoing research studies in acute and chronic pain.
Duration 12 months or more
Qualifications FRCA or equivalence; or FRCPC or eligibility; or completed specialist anesthesia training with MCCEE or equivalence
Additional Requirements Individuals with specific goals/interests would do well to begin discussion well in advance to permit adequate planning. Salary negotiable.

Pediatric Anesthesia Fellowship

Hospital McMaster Children's Hospital, McMaster University
Description 154 acute beds with 40 subspecialty clinics and one of the largest NICUs in Canada. Has 180,000 annual visits to the hospital.

8 Operating Rooms with additional anesthesia services provided for Diagnostic Imaging, GI, Interventional Radiology, Radiotherapy,
Oncology, and Neurology.

Surgical disciplines serviced include ENT, Urology, Opthamology, Neurosurgery, Orthopedics, Maxillo-Facial, Dental, Plastics, and
General Surgery. Cardiac Surgery and Organ Transplantation are not done.

Significant neonatal surgical volume.

Active Acute Pediatric Pain Service and Pediatric Simulation Program.

Rotations through NICU and PICU are also offered.

Active Department Research Program.

Cardiac Anesthesia Fellowship

Description Operating Rooms
20 weeks in the cardiovascular OR. It is the expectation that fellows are able to complete a minimum of 120 cases per year including: on and off pump CABG, valve repairs or replacement, minimally invasive procedures and other complex aortic repairs. On average 1700 cardiac cases are completed per year at Hamilton General hospital in the operating room with approximately 40% of these being valve cases.

Fellows are also expected to participate in procedures taking place in the heart investigation unit (HIU) including TAVR, Watchman procedure, and mitral clip procedure. Approximately 100-150 HIU procedures are managed every year.

Fellows will occasionally be assigned to elective lists in the electrophysiology (EP) lab including pacemaker insertions, arrhythmia ablations, cardioversions and other electrophysiological procedures that are considered complimentary to their training.

Echocardioraphy (TEE)
30 days of dedicated TEE days during a 12-month fellowship that are considered a core clinical skill. Training will be conducted mainly in the cardiac OR under the supervision of cardiac anesthesiologists. There could also be arrangements made to spend some time training with cardiologists in the echo lab, to complement operating room training.

Cardiovascular ICU (ICU-West)
The ICU West is a 17-bed unit that is managed by the department of Critical Care medicine in conjunction with cardiac surgery and takes only patients who are admitted by the cardiac surgeons. Anesthesia fellows are expected to cover 6 weeks of weekday coverage (Mon.-Friday 0800-1700). During this rotation, fellows will be engaged in the morning multidisciplinary rounds and participate in the workload as deemed necessary by the ICU coordinator. There will also be a total of 6 weeks of evening and overnight ICU coverage during which time fellows will remain in house and be under the supervision of the ICU staff. These calls will be distributed throughout the year.

Call Duties
In addition to in house ICU calls, fellows will be assigned a total of 6 weeks of home call distributed throughout the year, where they will be called back to help manage emergency cardiac cases that happen after hours. This would include but not be limited to: tamponade, emergency CABG or valve cases, dissections, endocarditis, etc. Should these cases occur late at night or in the early morning hours, the fellow would be relieved of clinical duties the following day.
Educational Curriculum Weekly didactic lectures and TEE reading sessions will be offered that cover topics in cardiac anesthesia and TEE. Those lectures will be prepared and presented by the cardiac anesthesiologists working in our department. Fellows will also be expected to research and present one cardiac anesthesia topic of their choice to the fellows and staff at our monthly rounds.

Fellows are also encouraged to attend and participate in all other department and interdisciplinary rounds. These include: TEE rounds, combined cardiac surgery, ICU, perfusion and anesthesia rounds, and departmental weekly rounds.
Duration One academic year: July 1 to June 30 (includes 20 vacation days and 10 conference days)
